Here are some instructions on how to prepare your forms in advance:
1. Click on iHealthRecord, on my home page, and create your confidential Personal Health Record. It takes only 15 minutes and helps ensure we receive accurate information about your medical history.
2. Print the iHealth Registration form and office forms at the end of this process and bring it to our office for your appointment.

Your iHealthRecord will provide a more complete and accurate view of medical history, medications, allergies and other diagnoses, so you don't have to fill out forms over and over.
iHEALTH MESSAGE CENTER
You can receive secure messages, FDA warnings and other educational materials in the MESSAGE CENTER. Log in at Patient Login on my web site to view the messages securely and confidentially.
iHEALTH EDUCATION CENTER
Created in partnership with the AMA, FDA, Centers for Disease Control, American Heart Association, and leading health experts, the iHealth Education Center provides ongoing health reminders and alerts about your conditions or medications. You subscribe to your areas of interest from trusted sources, and then access these messages at the iHEALTH MESSAGE CENTER.
